How Referrals Work
- Medicare participants suffering from a chronic health condition (3 months or longer) are eligible for rebates for up to 5 allied health services per calendar year (10 for Aboriginal/Torres Strait Islander patients) through a GPCCMP referral.
- Book an appointment with your doctor to discuss which allied health services best support your chronic condition. Ask them about including Exercise Physiology!
- Most allied health services will incur a gap payment, but not at Syndicate Health. We are a bulk billing practice so your referral would get your treatment plan underway with no out-of-pocket fees.
- Mention your Medicare referral when booking online or through our reception staff.
- Syndicate health is a collective. We aim to provide our support and services to many medical centers, allied health clinics, and orthopedic practices for their patients to receive expert care that keeps them informed about your progress.
- If your GP/specialist has spoken to you about a referral for exercise physiology, they can either write a private referral sent directly to our clinic or they can fill out the form at the bottom of this page.
- Our team will then contact you to book in your initial appointment.
- DVA members holding a White or Gold card are eligible for subsidised exercise physiology treatment of their accepted health conditions.
- Speak to your GP about a D904 referral to our clinic and mention your referral to our reception staff when booking.
- After your initial assessment and periodically throughout your treatment cycle, we will update your GP about your progress.
- If you have been in a motor vehicle or workplace accident and are in still in the process of rehabilitation, you will need to review with your GP to gain a certificate of capacity.
- During these appointments, you can ask your GP about a referral to exercise physiology to assist in getting you physically and mentally back to pre-injury levels of strength, function, and wellbeing.
- After your initial appointment, we will write a complete report to your GP and request funding from your insurer for further treatment.
